Courses for 2010/2011
iNSTRUCTIONS: Please fill out the online form below. You will receive confirmation of enrollment via e-mail, so please make sure your e-mail address is correct. You will need to fill out a form for each child you wish to enroll in courses offered by Grace Notes. To complete your enrollment, you will need to mail the registration fee for the class, which includes the supply fee to: Grace Notes Academy, 369 Bradfield Drive, Buda TX 78610. ***Make all checks payable to "Marie Cole," NOT to "Grace Notes."
**If you are enrolling your students for One Day Academy courses (Elementary Science), you MUST enroll them with One Day Academy, in addition to filling out the form below. The website is www.onedayacademy.com.
If you would prefer not to enroll online, please print this page and mail the completed page to Grace Notes Academy, 369 Bradfield Drive, Buda, TX 78610, along with the registration fee to secure your spots. ***Make all checks payable to "Marie Cole."

My child may participate in ALL class activities. Mrs. Cole and any class helpers will make every effort to conduct safe programs and events, but there is always some risk involved in science experiments and group activities. I accept these risks as part of my child's (or children's) participation and hereby waive any rights that I or said minor may have to hold Mrs. Cole, Grace Notes Academy, One Day Academy or hosting venues or any of their employees responsible for any and all injuries, damages, or losses sustained by the mentioned minor child while participating in any of the courses or programs offered by Grace Notes Academy, including events held at One Day Academy sites or in the home of Mrs. Cole.
Grace Notes expects a high standard of conduct and dress. Mrs. Cole reserves the right to ask students to change inappropriate clothing and to dismiss anyone for misconduct without refund. Tobacco, drugs, alcohol, profanity, disrespect and questionable remarks and gestures will not be tolerated. Although no dress code per se is in place, if a student is dressed in a distracting manner, he or she will be asked to change or the parents will be called to bring a change of clothing for the students. All students must wear close-toed shoes, such as tennis shoes, as part of standard laboratory safety gear. All long hair must be secured so that it does not accidentally get caught or dipped into an experiment. Students are encouraged to wear capri length or longer pants and shirts that do not bare the midriff as a measure of safety in case of chemical spills. Students need to bring a kitchen apron each week to class to be worn if needed. Any additional safety gear will be supplied by Grace Notes. Students that are not wearing clothing appropriate to a laboratory will not be allowed to participate in the hands-on portions of experiments.
Your medical insurance will be used in case of accident or injury, and all co-pays or other fees are the parents/guardians' responsibility. If you do not have insurance, all fees are your responsibility. You authorize Mrs. Cole, One Day Academy employees, classroom helpers or employees of host churches to attempt to contact you in a timely manner, to act in place of parents/guardians to secure proper medical treatment, hospitalization, injections, medicines, transfusions, and/or surgery in the event of an emergency. You also agree for the students to receiver over-the-counter medications (Tylenol, Benadryl, etc) according to label directions and to receive minor first aid from Mrs. Cole or any Grace Notes camp coordinator or employees of hosting churches or One Day Academy. **Note: We will not administer medications without a thorough attempt to contact parents or guardians first, and only in the case of true need, such as reaction to an insect sting. |
